Generalised anxiety: the worry with no single subject

Generalised anxiety is not fear of one thing. It is a scanning system left switched on, which finds a subject wherever it looks — the email, the mole, the child's silence in the car, the noise the car is making. Resolve one and the attention relocates within the hour, which is why reassurance provides such short-lived relief.

Treatment works on the mechanism rather than the topic. That means learning to recognise the scan as a process rather than as information, interrupting the rehearsal loop before it completes, and getting the nervous system out of a state of standing readiness it has been holding for years. Practical, unglamorous, and effective.

Relationship anxiety

Some anxiety lives almost entirely inside a relationship: reading tone in a two-word reply, the certainty that a quiet evening means something is wrong, the compulsion to check and then the shame about checking. It is exhausting for the person experiencing it and frequently corrosive to the relationship it is trying to protect.

Relationship anxiety usually has an attachment history behind it rather than a partner problem in front of it. The work separates the two — what is actually happening now, versus what your system learned to expect. Social anxiety

Social anxiety is often mistaken for shyness, but the engine is anticipated judgement, and its most reliable feature is the post-mortem — the drive home spent auditing everything you said. Avoidance provides immediate relief and steadily narrows the life, which is the trap.

Effective work is graded rather than heroic: small deliberate exposures, dropping the safety behaviours that keep the fear intact, and shifting attention outward during the event instead of monitoring yourself from the outside. It gets externalised as control — planning, managing, pre-empting, correcting — and the partner experiences that as criticism or distrust rather than as fear. Or it gets externalised as reassurance-seeking, and the partner slowly learns that no answer is ever sufficient. Either way, the relationship starts organising itself around the anxiety.

Naming that out loud usually changes the temperature quickly, because it reframes a behaviour the partner had read as being about them. From there the work can proceed on both fronts: the anxiety itself individually, and the accommodation pattern in couples sessions if both partners want it.

What type of therapy is best for anxiety?

There is no single winner, and the honest answer depends on what is driving it. Solution Focused work gives fast, practical traction on the worry loop and on avoidance. Psychodynamic work is the better fit where the anxiety is old, diffuse and tied to early experience. Rapid Transformational Therapy is often the most efficient option where the anxiety rests on one identifiable belief such as "I am not safe unless I am in control", because it targets that belief directly rather than managing its output. Most clients here end up with a blend, decided in the first session or two rather than chosen in advance.

How long does anxiety therapy take?

A specific, situational anxiety often improves substantially in six to twelve weekly sessions. Long-standing generalised or high-functioning anxiety is usually a longer piece of work, because the pattern is woven into how you operate rather than bolted on. Can anxiety be treated without medication?

For many people, yes — psychotherapy alone is an appropriate and effective treatment for a great deal of anxiety, and most clients in this practice are not medicated. That said, this is not a prescribing practice and nothing here is medical advice: decisions about medication belong with your physician or a psychiatric provider, and therapy works perfectly well alongside medication where you and your prescriber decide it is warranted. Severe or disabling anxiety is worth discussing with a physician in parallel with starting therapy.
